How Is Student Learning Determined and To What Extent?

The two-fold question above is- from the looks of it- straight forward and simple enough. Asking the question is one thing but answering the question though is something- in practice- very different and not as easy as it may seem. So how is assessment done? Like most ideas there is no singular method that usurps all other methods in efficiency and information procured. And like most things in life, including the multi-varied world of ideas, answering the question well and with meaningful information depends on numerous factors, both related and unrelated, both known and unknown (until of course discovered usually de facto).
