Request for Evaulation/Re-Evaluation of Credits - this form MUST be filled out and submitted to the Admissions Office in order for your transcripts to be evaluated.
NOTE: If the institution that you are transferring your credits from ONLY sends out ELECTRONIC transcripts, you need to have them send DIRECTLY to our institution. They can be emailed to: admissions@azwestern.edu Faxed or printed copies will not be acceptable.
