STEP-BY-STEP APPLICATION PROCESS

Application Process

1. Contact the Radiologic Technology office at (928) 344-7552 and ask to be placed on the Prospective Student List.

2. Visit the AWC library and read the current Student Guide, Applicant Information brochure, and view the video about the profession.

3. Complete all pre-requisites and CPT-R score by the fall semester prior to your intended application period.

4. Submit transcripts for evaluation for courses not taken at AWC.

   1. A “Request for Evaluation” Form must be submitted along with an official (sealed) transcript. Please contact Enrollment Services, by phone (928) 344-7601 or visit them in the 3C Building on Main Campus, or by mail: Arizona Western College, Attn: Enrollment Services, PO Box 929, Yuma, AZ 85366.

5. Attend a Mandatory information session. Students on the Prospective Student List will be contacted via email as to the date of the information session. Information regarding this session will also be posted on the campus calendar.

6. Submit a valid Fingerprint Clearance Card.

   1. If you have a valid Fingerprint Clearance Card, the original card must be submitted along with a completed application. Copies will be made by the Radiologic Technology Office.

   2. If you do not have a Fingerprint Clearance Card, you will have to apply for one.

      1. Application packets & instructions are available in the Radiologic Technology Office.

      2. Be advised that it can take anywhere from 4 – 8 weeks to receive your card from The Department of Public Safety. We will NOT accept applications without this card.

7. Submit a completed application to the Radiologic Technology Office.

   1. Applications are only available at the Radiologic Technology Office. Applications will be available January 14, 2013 – February 20, 2013.

   2. Unofficial transcripts and valid Fingerprint Clearance Card must be submitted with application.

   3. Unofficial transcripts must be obtained from the Admissions and Registration Office; printouts from Web Advisor will not be accepted.

8. Permission to take the HOBET exam will be given after CPT-R and Pre-requisite GPA are verified. HOBET testing will be available January 15, 2013 to February 28, 2013. A Study Guide is available in the Library Reserves.

9. Attend a 4-Hour observation at Yuma Regional Medical Center. Observations will be scheduled by the Radiologic Technology Office.

10. Sit for an interview with program Faculty and Staff.

11. Notifications of acceptance or declination will be mailed.
